Sunrise Community Mobile Health Van Helps the Needy in Weld County

The mobile health van is used for delivering services to migrant, homeless, and school based health clinics in Weld County. In addition, services are available to the under insured and those with no insurance. Overall, there are no boundaries to this vital health service. Reaching folks needing medical attention is its ‘driver.’

The mobile van offers free health screenings of vital signs including blood pressure checks and blood sugar checks. With a medical
provider on board, the Mobile Health Van is kind of a traveling Sunrise Community Mobile Health Van urgent care. Patients can be seen,
treated right on the van or receive further assessment with appropriate referrals and appointments made through Sunrise Community
Health in association with the Northern Colorado Health Alliance.
