Med Care Pro is a multi-use software sales platform that mainly for use by discharge planners to facilitate the process of patient placement when moving between medical care facilities. The primary patient movement occurs from hospitals to ancillary facilities, such as skilled nursing, long term acute care, home health, hospice, palliative care, etc. But, movement can occur between any of the facilities.
The program streamlines the process of matching bed availability and medical services provided based on minimal search criteria. Discharge planners should be able to ‘click’ on a facility and quickly see a list containing bed availability, service area by zip code, medical specialties, insurance providers accepted, or other information. The information needs to be kept current and/or updated by a representative(s) at the facility.
The program allows discharge planners to quickly assess potential locations, organize ever-changing facility services and availability, and ultimately save time for all users through information consolidation. More time is available to deal with difficult to place patients, preventing repeat visits to the hospital due to poor placement choices.
