This is the vision of Sharing the Care Campaign an empowering organization with a mission to improve the lives of Northern Colorado community members, particularly older adults, by identifying and respecting informed choices, promoting best practice in medical reconciliation and chronic condition management and communicating essential information at every transition. Their membership is desirous and actively moving forward to have a positive influence in the community through their collective impact. One aim they have is motivate patients and families to partner with their healthcare team.
